Before Lucky can visit Siobhan, Anthony visits her in her hospital bed.  Siobhan leaves a message.  Skye harbors Jax safely and in secret.  Carly faces off with Sonny for being a murderer.  Jackal, P.I., pauses over the bakery name “Luzetta’s.”  What is the gumshoe onto? Liz fears Lucky will blame her for some recent issues.  Will Lucky find Siobhan’s mysterious message?  Carly and Shawn discuss family as Sonny takes vigil in his greatest regrets at the GH chapel.  Sonny gets Michael on his side.  Maxie takes Jackal, P.I., to Georgie’s grave in hopes that will jog his true identity.  Abby calls Michael out on his irresponsible support of his father over Jax.

Skye delights when she discovers Tracy trying to cover her tracks about Gino Soleito.  Later, Skye meets up with Anthony to discuss the terms of their scheme as Johnny arrives to overhear.  Dante arrives to hear Sonny and Carly fighting about Jax.  An emotional glimmer of Spinelli shines through – and is gone again.  It gets ugly between Abby and Sonny.  Later, Abby runs into Johnny, and they discover they have a lot in common.

Despite his seizures, Jason tries to conduct business from his hospital bed.  Sam puts her foot down, and the two share a romantic moment planning their wedding.  Later, Robin arrives with a shocking explanation for Jason’s seizures.  Drugs threaten to take a place in Lucky’s life again.  Meanwhile, Steve questions Liz and Matt over missing drugs at the hospital.  Matt discovers Siobhan’s note and shows it to Lucky  – what does it mean?  Jackal, P.I., continues his stakeout of Luzetta’s, making himself at home in the dumpster.  Mac comforts Alexis as she mourns missing Jax.  Diane and Max break up.

At the hospital Patrick, Robin and Matt discuss Jason’s surgery. Robin would like Matt to assist Patrick, but Patrick bristles at the suggestion. He argues that his little brother would be too busy competing and would be a distraction. At the nurses’ station Elizabeth frets that Steve and Epiphany will discover she was the nurse who stole the drugs. Matt calms her down; she got them for Lucky, so she shouldn’t worry. Carly catches up with Michael to tell him that Sonny is manipulating him, but Michael accuses her of being one of her “I hate Sonny” rampages. He knows Carly will forgive Sonny sooner than later. Carly admits that was true in the past, but says this time is different. Matt and Elizabeth join Robin and Patrick as Patrick repeats that he does not want Matt assisting him in surgery. Matt argues that Patrick needs an extra pair of hands and eyes and Elizabeth agrees.  Meanwhile Jason and Sam debate on whether Jason should have surgery. Jason wants to make this decision with Sam; what does she think? Do they risk the time they have together now or try to buy more time later? Later Sam joins the group by the nurses’ station. She announces that Jason has decided to have the surgery. Carly overhears and slips away as Monica tells Sam that she made the right decision. Patrick and Matt continue to argue until Matt angrily agrees to stay out of the OR.

Carly sneaks into Jason’s room where she can’t help but dump her worries about Michael following in Sonny’s footsteps. Sam enters and chastises Carly for making Jason worry before his operation. Carly realizes Sam is right and leaves. Sam takes a moment to thank Jason for every day they had together and how she looks forward to the rest of their lives together. She leans down to kiss him, but a nurse interrupts. It’s time to head to the OR.

As Jason is wheeled away, Robin urges Monica, Carly and Elizabeth to not watch the surgery. Instead go home or somewhere safe and think of Jason. Patrick warns Edward that Jason may not wake up the same person, which gives Monica hope. She wonders if it’s bad to wish for the old Jason back. Edward grimly warns Monica to be careful for she wishes for.  Sam returns to Jason’s hospital room where she looks at a photo of them from their vacation in Hawaii. She has a fantasy where she and Jason are married and have moved to Hawaii. Jason spends his days surfing while Sam searches for sunken treasure. Her latest adventure includes a tribal mask that leads reveals the coordinates of an old World War II ship (that is rumored to have gold) when placed over a map of the area. Jason excitedly agrees to go with Sam as a stranger lurks nearby, listening. Elizabeth heads to her art studio where she remembers Jason seeking shelter there after he had been shot (many, many years ago). She has a fantasy where Jason is an architect and she is a successful artist. Jason announces he has to move to Tokyo for his next project. Elizabeth agrees to go with him as we learn they have no kids and live a very bohemian lifestyle. Jason urges Elizabeth to finish her last piece for her upcoming art show as he removes his clothes. It turns out Jason is also Elizabeth’s muse. Later Jason finds Elizabeth’s unopened package of birth control.

Monica returns home where she gazes at an old photo of Jason. She has a fantasy where Alan (who is alive in this alternate world) brings Jason into the living room. Here’s to the new Chief of Staff! Tracy and Edward join them in a toast as Monica subtly removes the champagne flute from Jason’s hand, indicating he could have a substance abuse issue. Sure enough, later Monica sees Jason snorting cocaine. Carly enters Jake’s where she has a fantasy of Jason as the new mob boss. He enters Jake’s to play some pool with Carly. Jason tells Carly he couldn’t have secured all those territories without her and almost kisses her. Carly smiles; the sky is the limit as long as they are together! Later Michael walks in and barks at his mother. Back at the hospital Patrick goes to remove the foreign particle in Jason’s brain, but he begins to bleed.
